Strategic Plan Worksheet – Program

Goal #1:

Reform the service planning model.

  1. Continue and refine the outcomes performance measures
  2. Influence state policy
  3. Expand unpaid relationships for the people we serve.
  4. All PMs will have the same job description as Lisa.
  5. Include a focus on community volunteering.
  6. 6 Bring day center activities into compliance with the Center for Medicaid Services (CMS) integration mandate.

Goal #2:

Provide competitive and integrated employment for people we serve who want it.

  1. Continue the community action team (C.A.T) model in both counties.
  2. Secure “legitimate community employment” for four individuals in each county.
  3. Expand relationship with Opportunities for Ohioans with Disabilities (OOD) in each county.
  4. Maintain our CARF accreditation.

Goal #3:

Support self-advocate leadership.

  1. Fund at least one consumer for Steps Toward Independence and Responsibility (STIR) training in each county each year
  2. Fund staff mentorship to STIR and People First activities
  3. Assure consumer representation on the board, per the bylaws.
  4. Represent Havar on Ohio Self Determination Assoc (OSDA) board

Goal #4: 

Pursue accessible, affordable housing for people we serve.

  1. Work with OPRA to advance the Athens Co. partnership toward development of an “autism home”
  2. Highlight employment as a way to increase earned income.
  3. Ask Athens Metro Housing Associaiton (AMHA) to identify appropriate housing resources for our consumers.
